Welcome to the wonderful world of B.A.S.E.'s website!
We hope this website will make it easy for you to check in on the exciting things going on at BASE, and inform any interested families on our program. On our "Know-It" page, you can find any important posts and view our monthly or weekly bulletins. A coaches page can also be found where you can learn a little about the staff that serve students K-7th grade and help them have a fun time at our facility. To download enrollment information scroll down to the bottom of this page and read the instructions to ensure that you download all the correct paper work.The BASE staff also encourages you to drop by to visit today for more information and/or questions. It is suggested that you call ahead for appointments at 417-777-4810. We hope that you will consider enrolling your child into our unique program!

BASE Ministries is an after-school program that seeks to assist families by providing positive and enriching after-school care for K - 7th grade students in a Christian atmosphere. We are open everyday that Bolivar Schools are open from 2:50-6pm, and also offer a summer day camp during the summer break. Through our summer program we offer summer school care (including drop off to and pick up from each school), and all day care during the regular summer break weeks. We have weekly field trips to exciting places including Play 4 fun, Springfield Zoo, and the Springfield Cardinals Stadium. We also have weekly swim days, visits to the local park, skating, and watch movies in our private movie theater! We provide morning and afternoon snacks including equipment for our many group game opportunities. Our prices for the 2010 summer camp is $90 for the 1st child and $80 for 2nd child. Part time prices are available as well as drop-in slots (Prices vary). Drop by today to pick up an enrollment packet our beloved summer camp!

Enrollment forms for summer & school year:
Click on the links below to download our complete enrollment packet and forms to enroll your child into our 2010-2011 after school program or 2010 summer camp. Completed forms must be turned into the BASE office before your student can attend, as well as a $15 enrollment fee. Shot records are also required to be submitted to the BASE office before your child can attend. You will also need to fill out a small emergency card when you come in to BASE with your enrollment packet. You will receive this form from the BASE director and any person who is allowed to pick up your child(ren) must be listed on it. Our prices for the 2010-2011 school year are $25 for full time, and $7 per day for part time. Drop-in status is also available for $8 per day and no enrollment fee charge. For summer enrollment you will need to download our summer enrollment packet, summer enrollment form, Medical form, and the NPR. School year families also enrolling in summer are subject to only a $5 enrollment fee. Thanks and let us know if you run into any problems!
Summer enrollment packet- This packet is unique to our summer program, though still listing our rules and policies. It is each families responsibility to read through the packet in order to understand what is required of them and BASE while their child attends.
Summer enrollment form- This form is unique to our summer program. Downloading and filling out "school year enrollment form" is not needing for students enrolling is our summer program only.
School year policy guide- This packet has all of our rules and policies listed for the upcoming 2010-2011 school year. It is each families responsibility to read through the packet in order to understand what is required of them and BASE while their child attends.
School year enrollment form- This is our basic enrollment form. Addresses and phone numbers for each individual must be filled out COMPLETELY! This is required by the state of Missouri and it also helps us to get a hold of you quickly if we need to.
Medical form- Please list any of your child's allergies or medical needs on this form before turning it in to the BASE office.
Notice of Parental Responsibility (NPR)- This form details out some more of BASE's policies that are important for each family to know in detail. Signature stating your read the form is required.
Agreement of release- This form is for parents to sign stating that they have read, understand and agree to BASE's policies and procedures. Also by signing your family agrees to follow BASE's rules and regulations.
