At First Baptist Church we use an annual Medical Release/Permission Form which covers about 90% of our youth ministry events and activities.  Once this form is completed and returned to the youth ministry office, it is kept on file so you only have to fill it out once.

If your son or daughter is taking part in a youth ministry activity or event you can print out the form out below and drop it by the church office or you can fax it to the church at 417-777-4706.

 A seperat release from is used by Lifeway for Centrifuge and must be completed and notarized by camp.  When that form is available we will activate the link.
